Gym Membership
Advanced Care Physical Therapy, Aquatic and Fitness Center in Niagara Falls, New York looks forward to having you join our host of satisfied clients. Please remember that the fitness center is located in Niagara Falls, NY and is not available at our Wheatfield location.
Our 2008 rates are as follows:
$19.00 for 24 months
OR
$29.00 for 12 months
* Each package requires a $99.00 enrollment fee.
For an additional $30.00 we also provide a specially designed key which allows you to access your current training program and see your progression as you utilize our beautiful Technogym equipment.
For your convenience, below are the forms that you may have to fill out when you come in for a gym membership. To access a form simply click on the link and once the form opens you can print it, fill it out and bring it with you to our facility or send it in by fax. Children under the age of 18 must fill out the child release and waiver. Our fax number is 716-285-1281.
